Seeking Friendly compassionate Travel Nurse Practitioner/PA

Location: Houston, TX

Mon-Fri Days, No Call

Responsibilities:

• Assist care coordinators across the different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patient condition and reporting through our proprietary software system
• You will be cross-trained in other areas of care management
• Gathering and analyzing patient information
• Detailed review of the system
• Review of all pertinent patient labs, studies, and provider notes

Requirements:

• Nurse Practitioner License
• DEA License
• 6 months experience in long-term care 
• Reliable transportation
• Must be willing to travel to different Facilities within the North Houston/ South Houston Areas

Salary Range: $150,000.00 - $200,000.00
